Default Laid Off During Disability Leave

My question involves labor and employment law for the state of: CA

I was laid off after being put medical disability at 30 days. My employer replaced me with the Asst. Manager and hired the same amount of staff as if I were there. I believe they decided this to save them money, but not due to lack of business. The business had been thriving since I started nearly 5 years before. Is this legal to do, and aren't they required to maintain my health insurance for a period of time? I had my own insurance that they reimbursed me a certain amount per month. Oh, and when I contacted my employer that I was ready to come back she told me there were no open positions even though they did not replace the assist. manager job per say, but had the same or even more employees than when I went out.
