    If you were not on the employer's health plan, then you are not eligible for COBRA and no penalties apply.

    There is no requirement that 401k information be sent at any particular time. 45 days may have been the rule where the person who told you that worked, but it is not a law and there are no penalties involved.

    Truly, I'm not seeing anything for which you are due any monetary penalties.
