    should have been at the birth .. the nurses usually input this information ... likely you could have had your name put on at that time

    if I establish paternity at the hospital, and sign the birth certificate, will he automatically (and legally) have to have my last name, even if she protests?

    No. He will not. He will have whatever name Mom chooses to give him. No law in Missouri (or any other state) will force an unmarried mother to give the child the father's last name regardless of what he signs.
