Default Getting a Passport when Birth Certificate and SSI Have Different Info

My father is trying to get a passport to travel to Mexico. He was born in California under the name Eberardo. While in school his teachers could not pronounce his name, so his 3rd grade teacher started calling in Everett. This was in 1940's in California and not many bilingual teachers. So it stuck, his license, social security card, marriage certificate all have Everett. But somewhere along the way his birth year was changed. His birth certificate says 1934, and his drivers license has 1935.
When he applied for a passport , he was denied for to many discrepancies. He is the same person. What should he do? Which legal documents need to changed?

He needs to get the erroneous errors cleared up. He has a bigger problem. He is not entitled to collect SSI if he leaves the country for more than 30 continuous days.
