    Question Changing a child's name on a birth certificate

    My daughter lives in Arizona and is married to a man who has 2 last names When she delivered my grandson she gave him the last name of her soon to be husband to only find out later that she used the wrong last name. How can she fix this problem before her son goes to school? Does anyone know how much something like this would cost? Thank you for ur time and patience.

    There's a sample form for petitioning for a name change for a minor here. You can also check with the court clerk's office for your county to see if they have a standard form. It appears that the filing fees are $130, although that may be different for a minor.
