has he been proven to be the father through the courts?
has he signed a acknowledgement of paternity?
yes and she's is 4 months old and we had a paternity test done through ors

If you have a court order declaring him the father, you file to amend the birth certificate.
If you do not have a court order, then you would have to file an Voluntary Declaration of Paternity and with that, file to have the birth certificate amended.
Not positive on the name change but I believe you will have to file for a name change through the courts
