    Default How Do I Change My Child's Surname

    My question involves name change laws in the State of: Rhode Island

    My daughter is four and when she was born, my husband and I were not married, so I asked that her last name be my maiden name. She was born in Illinois, but we live in Rhode Island now. How do I go about changing her surname to my husband and I's last name? He is her father on the birth certificate. There's no issue of paternity.
