You told us before, "I was never given a form to fill out". His not giving you a form is different from his stating that no form exists. Which was it?

You stated that he told you "that he would ask his supervisor about it." Are you telling us now that you spoke with the supervisor and that the described conversation never occurred? That you spoke with the supervisor and were told that the waiver was approved? Because if the supervisor said "no", it is understandable why your parole officer wouldn't commit insubordination and risk discipline or termination by signing the form.
