The chances that he is going to be in the same room, with his gun, as a man who is raping a woman, and the man continues to rape the woman while the guy with a gun is standing there, AND the hero with the gun knows for sure that it's a rape situation AND the woman doesn't start strongly fighting back because she knows she is now backed by the hero with the gun ... are pretty small. Also, there is a difference between defending someone else ... and retaliation.
There are also cases in which women having consensual sex have said that it was rape because they were caught at it and didn't want to look bad/adulterous. It comes out later in the wash. So you have to be very sure of what is really happening if you want to shoot first and ask questions later.
