Default Name Change for my son

My son is almost 15. His father is on his birth certificate, pays child support, and has visitation. My son has my maiden last name. He wants to change it to his father's. I have no problems with it, nor does his father. Where and what would I start to do this? I reside in Missouri, his father resides in Colorado. My son was born in Colorado. Any information would be helpful.

If everybody is in agreement, you should be able to do the name change through a consent order with the court which has jurisdiction over the custody matters.
