    Thumbs up How to Hyphenate a Child's Name

    My question involves name change laws in the State of: Oklahoma

    My son is 5 and has not had contact with his father in over a year. My husband is going into the Coast Guard, and I want to change my son's last name to either my name (which is not the same as my husbands, if that matters at all.. I like to keep things difficult) only or a hyphenate of my name and his legal fathers last name.

    I have personal reasons for wanting this done and I think it would be easier when filing for military benefits such as insurance and housing. Is this something I could do without a lawyer as long as his father agrees? Could anyone direct me to the forms I would need to fill out or at least which court to call for more information? Thank you very much for any help!

    If both you and the child's father are in agreement, the process is pretty simple - a link to the statutory instructions and a sample petition form can be found here.
