    Default Estimated Costs Fee

    My question involves insurance law for the state of: Arizona

    My daughter is scheduled for a procedure tomorrow and we have insurance with no deductible or co-pay. The office where the procedure is at called today and said that they are going to reschedule her if we don't pay a 10% estimated costs fee tomorrow before the procedure. This is even though it is completely covered by our insurance.

    What is this and is this legal?

    Any help would be greatly appreciated.

    I suggest you speak with your insurance company. I suspect the hospital has been informed you do have a co-pay of 10%.

    No, even the lady on the phone said I don't have a co-pay and that this fee wasn't a co-pay but an estimated costs fee. I've never heard of that before.

    So, what was the answer from the provider when you told them you do not have a co-pay nor deductible to meet and that their fees and costs are covered 100%?

    To be honest, my wife talked to her, and she didn't ask I guess so I will find out tomorrow. I was just hoping to have some research to back me up if this is shady.

    It really sounds like the provider is under the impression you do have a co-pay. I can see no other reason, immediately, why they would demand the payment.

    but to the more basic question; a provider can refuse to provide services pretty much as they choose and if they believe you are liable for some portion of the costs, they most definitely can refuse to continue until you do make such payment.
