How many employees are employed by the company you work for?
How many hours a week do you work?
How long have you been employed by this employer?
Doe's your employer have multiple locations and if so how far apart are they in miles?
If your not at work, doe's it hurt your company?
I'm not looking to start an argument here, I'm mainly trying to save the OP from telling the employer there health information. The OP may not even qualify for FMLA in the first place, and if they don't qualify, there is no reason to give a note that implicates there health history.
