    Default My Latest UI Check Has No Claim Form

    My question involves unemployment benefits for the state of: CA

    I have started work part time and filled in the income and place of employment on my previous claim form.

    I just got my UI check on saturday, yet this one has no claim form attached to it. Does UI end if my income exceeds a certain amount during a claim period? Apparently, that is the case as there is no other reason for there not being a claim form, as I turned one in at the appropriate time as always. The form states that the benefits period ends on the same date as stated on previous claim forms, and that the amount remaining on the claim is what it should be, based upon what I have just been paid, yet no claim form.

    What is going on?

    You can request a replacement form here - choose category Unemployment Insurance Benefits, then choose topic Claim Form - Need Replacement Form.
