    Question How to Fill Out Form 1203.4 from the California Expungement Packet

    My questions involve expungements or pardons for the state of: California

    I need help accuratly filling out the 1203.4 form.
    My misdemenor was in 2005, i have completed my probation and payments, and have moved and no longer have my paper work.
    I do have a copy of the record from the county website.
    this is my only violation

    In the box to the right of the PETTITON FOR DISMISSAL FORM and just below the box that says for court use only is a box that says CASE NUMBER.
    - is this my case number, or is this to be filled in by the court for the dismissal?

    In the box DEFENDANT'S INFORMATION, the first entry is "CII:"
    -What is this?

    1. On (date)__*__ the defendant in the above-entitled criminal action was convicted of a violation of section(s) (specify):__**____of the (specify):__***__
    -* do they want the Filling date or the Disposition date?
    -** how do i determine the section is that the charge? PC 487 (B)(3)?
    -*** what code?

    lastly the filing requirements want "Proof that full payment of court ordered fees; fines, and victim restitution applicable to the case(s) have been made."
    -where do i get this proof from? I lost my paperwork when I moved but I do have a copy of the report from the county website including my XREF#, case #, court ID#.

    Yes, the spot for the case number is in fact for the case number.

    CII = California's Criminal Identification and Information number.

    Where they ask for the date of conviction, they want the date of conviction. Where they indicate they want the code section, that's where they want the code section. Where they indicate that you should identify the code (Penal, B&P, Vehicle, etc.) - that's where you identify the code.

    You can inquire about obtaining copies of documents from your court file with the court.
