If it is a public clinic, check with the government agency / unit of government which is responsible for the clinic. You may find that its budget is already publicly available. If not, you can make a request under Florida's Freedom of Information laws.
I was hoping to find a specific request form. Since this clinic gets funding from county, state & local sources, would I just make the request directly to the CEO of the clinic? I am particularly interested in salaries & other expenditures.

You can ask if the clinic (or governmental unit/agency responsible for the clinic) has a FOIA officer. They may offer a form; you may also be able to find a sample Florida FOIA form with some Internet searching.
