Default Can a Parent Have More Than One Name Listed on a Birth Certificate

My question involves name change laws in the State of: New York
I currently have dual citizenship with Italy through my grandparents. I was born in the US. In order to renew my Italian passport I have to register my children with the consulate (they are minors- Italian law will not allow me to just register without registering them). The problem is that my Italian passport has my birth name on it. I had my birth name (first and last) legally changed prior to my marriage, their births, and subsequently my divorce. The Italian consulate does not recognize legal name changes - at all, not even worth questioning, trust me. In order to prove my children are mine I need to add an aka to their birth certificates for my name (it currently has my legally changed name). Is this possible? Or do I get an addendum? If so, how do I do that? I do NOT want to reverse and legally change my name back to my birth name. I have the original court decree with my name change and seal. I have my original birth certificate with my birth name ( I never filed to change it). But, the only thing the Italian consulate will allow is something that shows somewhere my birth name on my children's birth certificates, (and my marriage and divorce papers). I don't think an affidavit will just do. Help!

You need Italian law, not US law.
