    putting father's name on birth certificate without paternity
    I'm due to have my baby in july. The child's father and I live in 2 different states. He's in Florida and I'm in New Jersey. My question is if he's not able to make it in time to sign the birth certificate in the hospital, would a notarized letter from him stating that he his the father before I give birth be enough for me to put his name on the birth certificate. Our main concern is that the baby comes home with his last name.

    You should be able to get a "Certificate of Parentage" form from the New Jersey Child Support Office, which is the form you and he would complete to voluntarily acknowledge paternity.

    would this form be able to be signed before the birth or would I have to wait until after the birth?

    You may be able to execute it at any time, and file it after birth. The Child Support Office should be able to instruct you.

    i went to the new jersey child support website and it states that the cop has to be signed in the hospital. Is there anything that can be used to show he actually acknowledges paternity before the paperwork is filled out for the birth certificate?Please help!
