    Changing My Daughters Last Name to My Name
    My question involves name change laws in the State of: TENNESSEE
    I would like to know details on how to change my daughters last name to my last name. I was never married to her father, but his name is on the birth certificate. She was born in 1995 and out of those 13.5 years he has never been around on a regular basis, and that has been his choice. He moved to a different state about 3 years ago and he has not contacted me or her since. He never paid child support on a regular basis, last payment was in 2007. The State is not having any luck finding him to enforce payment, but that is another case. My daughter will be starting high school in the fall and she wants to have my last name before school starts. She doesn't like having a last name that is different from everyone in her family. No one in his family keeps contact. Please advise me on the steps to take.

    You may petition the court for a change of name for a minor.

    Shelby County offers a form online. You can obtain the forms and instructions for your county from your local Clerk of Courts.
