My question involves name change laws in the State of: Vermont
This is actually an easy one, because my wife and I are her biological parents. My daughter was born in Mass. My wife and I were not married when she was born so she took my wife's maiden name, as I anticipated that we would go our seperate ways. her name appears that way on the birth certificate and Social Security card. We are both the biological parents, and been married since then, and I've always meant to correct the name since it will run into issues down the line. We now live in St. Albans, Vermont. I've been told by courts in both states that I need to go to the other because she was born in Mass, her birth certificate was issued there, but we live in Vermont and a name change needs to occur in the state that you reside.

Contact the Massachusetts Department of Vital Records, and ask them if they will change the name on the birth certificate based upon your presentation of a certified (or exemplified) order of name change from Vermont. If not, you may have to change the child's name in Vermont, register the order in Massachusetts, and then proceed to have the birth certificate changed based upon the Massachesetts court's formal recognition of the Vermont order.

OK so I first need the "Order of Name Change" form, (where can I download that from) have the completed form certified by a judge (?) and then present it to the Mass Dept of Vital Records so they can make the update.......

Mass. Vital records tells me that: Statutes do not allow for amending birth records to reflect legal name changes. For identification purposes, you will need both the name change certificate and a certified copy of the birth record.
So her birth certificate can't be changed, I guess I don't have a problem with that. But for identity purposes, we can go through the name change process in Vermont to get the reissued SS card? Is that correct?
