    Where Can I Get Guardianship Forms?
    My question involves guardianship in the State of: Indiana - Lake County

    My 14 yr old cousin lives with me, I've had temporary guardianship since April of 2008. My school district won't let her go to school unless I have a court signed document stating that I have guardianship, I have the temp guardian papers noterized and another paper that the lake county school board said was ok, but my district says they are not. She hasn't been enrolled in any school since April, and school is about to start again and I want her in it this year.

    The parents are willing to give me guardianship, but going through a lawyer costs like $1200 and we can't afford that, but make too much money for a pro bono lawyer.

    Since it is a no contest (both parents are willing to sign over guardianship) case, do I have to have a lawyer?

    and if not, does anyone know what forms I need and where I can get them?

    To find out if you can represent yourself, you can contact the court, or try the Self-Service Legal Center. Either one should have the forms:
    http://www.in.gov/judiciary/selfservice/

    Another option is:
    http://www.kidsvoicein.org/

    If you google "indiana courts guardianship forms" (without the quotation marks), you should get a link with this address:
    "http://www.kidsvoicein.org/documents/Book/Appendix/new/Appendix5GUARDIANSHIP%2520FORMS.pdf"

    You can't copy this into your browser, so you'll just have to find it through googling. Anyway, the .pdf has the court form that I think you will need.
