    My question involves name change laws in the State of: Indiana

    I never changed my last name with social security when I was formally married. Several years after my divorce, I wanted to go by my maiden name again on bank accounts, bills, etc. That was easy to do since my social security card has my maiden name on it. I lived in Ill for many years, and used my my married name on my drivers license. I had lost my social security card but a dvm employee let it slide to get my drivers license under my married name.

    Now I live in Indiana. I have a up to date drivers license from IL with my married name on it, and all other documents are in my maiden name. The DVM wants some kind of name change certificate, but I legally never changed it to my married name in the first place. In Indiana, they refuse to issue me a drivers license even though my bills, bank card and social security card are all in my maiden name.

    Have you tried taking in a copy of your divorce decree?

    My divorce papers have my married name on each page. I wont go into detail as to why. The lawyer who drew up the papers took information from my drivers license.
