Hyphenation Of Minor Child's Name In Florida
Just a couple questions...I've read through some of the name change questions...my case is no where near as bad as some! At birth, I gave my son his father's name. His father has regular contact and pays support. No real problems between us. I've never been married...and I would like to give my little guy, who is now 5, my last name as well. So that he carries both mine and his father's last names. His father agreed to this SEVERAL times, but now is saying NO! I would assume that would work the way any name change would. I would petition the state. I don't think I'm doing something crazy. I'm not asking to remove a name. I would like for his to carry BOTH, since he LIVES with me! When and if I get married, I'll keep my maiden name and drop my middle so that we still have matching names. Just wondering how tough this will be since the father is wanting to fight me? I can tell you, because of this, I will not be making it easy on him. He doesn't have court ordered visitation and I will not be nice any longer. He made a promise that he has broken and I don't appreciate being lied to. Will I need his father to sign something or can and will the courts override this since I'm not being unreasonable? Will he be notified by the courts once I file? I'm not sure how any of this works. Can I print the paperwork online or do I need to go to the courthouse? Does anyone know what the fee(s) are to complete this? Thanks for your help!

You can get the forms here.
If he does agree, you can use packet #24.
If he doesn't agree, you will use packet #25, and you would have to have him served with the petition.
Call the courthouse to find out the cost of filing fees. Or research your county courthouse online and see if that information is available.

Thank you. I'm guessing I'll be using packet #25! There is actually a fee listed on the packet information. I'm wondering if that is a standard fee. Has anyone had experiance with this? Wondering what my chances are...

You can confirm the fee with your county court.

Because you are wanting to hypenate the name, you'll have a very good chance at succeeding because most Judges in Florida love to hyphenate names.
