You would need to file a motion for extension of time -- that's a motion that must be prepared, not a standard form. A debtor in this position really should be thinking about hiring a bankruptcy lawyer.
Chapter 13 bankruptcies are difficult for most debtors to handle without a lawyer, and the fact that the debtor is in this position is a pretty good indication that the debtor is going to have more problems along the way if not represented by a lawyer.
