    How to Change a Child's Last Name With the Other Parent's Permission
    My question involves name change laws in the State of: Kentucky

    I was wondering what court would be like in order to change my daughter's last name to mine, instead of her fathers. I have the fathers permission, but will I still need to prove it is in the best interest of the child? Or will they grant the name change with no problem, since both parents agree? Thanks in advance.

    If both parents are in agreement to the name change and join in the petition it should not be a problem. The Kentucky Courts have a form for this: Petition for name change. You’d fill it out and both you and the father would sign it. Then file it with the appropriate court.
