What happens if a person who is on probation refuses to sign a document presented by his probation officer, setting forth rules that he is supposed to follow while on probation? Can he be violated for breaking rules that he did not agree to follow?

If this is a document that he is required to sign simply to begin his probation, outlining the terms of his sentence with the court, then he should expect that he'll end up back before the court and either be instructed to sign or re sentenced to jail.
If he violated his probation and the probation officer is presenting him with rules that she wants him to follow in order to avoid a probation violation proceeding, then it is likely that a refusal to sign will result in his being brought up on probation violation charges for the conduct that led his probation officer to make the demand.
If these are new rules that are not required by his sentence, and there is no issue of probation violations, then there is no immediately obvious reason why the probation officer could require him to agree to the rules; and the rules would not of themselves change what behavior is or is not a probation violation.
