    Serve the father or any prospective father of the child. Both parents have a right to be involved.

    I want to change his name to his bio dad..I just picked a name when he was born.we were split up and I wanted nothing to do with him..how do I put his bio dad name on birth certificate

    If there is no legal father, then you can choose a surname when you establish paternity, by making that request on the AOP form. If there is a legal father, things become more complicated.

    I'm not understanding you...he doesn't want a paternity test he knows jax is his...

    You have to file a parentage suit in court in order to do this. He may know Jax is his but he needs to establish paternity. The court would have to establish parentage and then they could order the child has dad's name. AOP cannot be used to CHANGE a child's name.
