My question involves juvenile law in the State of: Minnesota
I have gone over the Minnesota law and the courts will review a "Petition for Emancipation" ,but will not assist you in the process. So my question is can anyone tell me where I can find an emancipation form.

Minnesota has no hard and fast emancipation law. Each case is reviewed individually. If your parents kick you out or you run away and they don't make you come home and you can prove self sufficiency you might have a very, very, minor chance.

Why do you need to be emancipated? I live with my mother and she verbally and emotionally abusive. I am also strongly against her religion which she insists on pushing on me. -

Given that MN does use a case-by-case method, you'd generally have to be living outside of the home and be capable of fully supporting yourself.
Full market rent, utilities, health insurance, transportation, food, clothing - you get the picture. -

What if I were to move in with my Father who lives in Maine and he gave consent to be emancipated. Could I then move back in Minnesota as an Emancipated Minor?

So dad is willing to sign papers to emancipate you (which he can't give consent to) therefore absolving himself of the legal and financial responsibility of supporting you instead of having a conversation with mom about a custody modification.... I suppose no one has thought of that. Yeah, he's got no motive. Once those papers are signed he can decide he doesn't want you there anymore and then what?
But luckily, he CAN'T give consent for you to be emancipated so I guess you're going to have to live with mom because none of the reasons you've given, coupled with your inability to support yourself means your petition would be denied anyway.
