    My question involves name change laws in the State of: at birth my daughter had my last name because her father was locked up, but when he got out I changed it to his last name on the birth certificate but I didn't change it on her ssc or her insurance and everything else I have off hers, but now her father dont want nothing to do with her, and I want to get her last name changed back to mine how do I do that? How much will it cost?

    We can't tell you how much it will cost. Call the court to find out.

    Not without going through the proper channels. You did the process once already, try it again.

    I suspect that they achieved the name change the first time through acknowledgment of paternity, such that this would be a different process.

    She chose not to follow the instruction to identify her state so there's little that we can say. Many states make available information and standard forms for name changes.
