    I don't think you're understanding my point.

    My point is, you have been given a generic document that we cannot read. There is no specific document that is uniformly used in these situations.

    It could be hand-written on a napkin for all we know.

    Right, exactly! So you need to ask the hospital exactly what the document means. Ask for clarification.

    Are you not reading anything - the form, or what you've been told?

    We cannot explain to you the terms of a form we have never seen. It is impossible. If you believe otherwise, tell me what book is sitting on the table by my right arm, then tell me what it says on page 43.
