Submitting Modification Forms Without Custodial Parent's Financial Info

My question involves child support in the State of: NJ

I am filing for a modification of an existing support order in NJ. CP is not cooperative and will not provide an updated CIS or any financial documentation. The paperwork that is supposed to be included in my motion for modification requires both our financial data in order to calculate the amount of support.

If I don't have CP's numbers, do I leave the form blank and ask the court to compel CP to provide, or do I enter (and label it as such) estimated numbers when I submit my motion papers?
