    Changing My Daughters Last Name to Mine
    My question involves name change laws in the State of: Indiana

    Hello there. I have a child who will be turning 10 next month. She is disabled, and has moderate/severe autism with cognitive disabilities. Her father wanted nothing to do with her after her diagnosis eight years ago...I get child support, but it is very irregular. My daughter has started writing her name with my last name. Her father is not in her life, and wants nothing to do with her. He has other children who are "normal" and doesn't even claim my daughter. How do I go about getting his parental rights terminated, and changing her last name to mine? Is there even a way to do this?

    Changing the name and terminating parental rights are two different things.

    The name change is easier, all you need to do is get him to either consent to it: http://www.in.gov/judiciary/selfserv...r-consent.html or you go to court and show the abandonment. If you click on the links on the left edge you'll probably find many other pages of information that will be interesting to you.
