    Adding (Not Changing) My Last Name to My Child's
    My question involves name change laws in the State of: North Carolina

    When my daughter was born almost 2 years ago, I filled out the worksheet so that she could have her father's last name. He was not able to be on the actual birth certificate since he died 2 months before the birth, and we were waiting until after the birth to get married. I thought having two last names would make her name too long (it was stupid I know, but I struggle with this issue), therefore I only put her father's last name. Now, I regret it. Can anyone help me as far as adding on my last name to the end of her name? (Where to go, costs, process in general, etc. )

Neither the ten (10) day Notice posting nor Affidavits of character are required for name changes of a minor child. Please bring the birth certificate and identification of the parents.

    I would not be at all surprised if your county court has a standard form you can use to petition for the name change.
