Correcting Mother's Name in a Child's Birth Certificate
My question involves name change laws in the State of:Baby born in NY, currently residing in Alabama.
My husband while filling the application for our daughter’s birth certificate, accidently filled my name wrongly. Instead of reading Mary Jane Hathford it was entered as Mary Hathford Hathford.
We tried correcting the name through the hospital and at the Birth Registry Office. We provided my Passport and Birth Certificate as supporting documents.
They rejected them saying they do not specify my first, middle and last names.
The rejection letter says to change the mother's name legally in the local civil court. We recently moved to Alabama and are not sure how to go about it.
Please advise, the baby is 1yr already and we still have to apply for her passport.

You have no identification whatsoever that documents your full name? Your middle name is not on your birth certificate, driver's license, passport, or any other document? (If that's truly the case, it would seem that from a legal standpoint you don't have a middle name.)

Thank you for your response. My birth certificate and passport 'do' have my middle name but in my country , they do not specify first and middle name ,instead it just says given name.
I did see the process to change an adults name in AL but I thought it was not applicable to me.
So, any advise?
