How to Complete a Case Information Sheet For a Protective Order Appeal
My question involves restraining orders in the State of: California
Hi, my name is Hikaru (alias.) I'm 20 and in Arizona, Yet due to some circumstances surrounding trying to help a friend, and because of his psycho mother, I was served a restraining order across state by her.
I went to studied how to fight it, sent in my answer forms, went to Cali for court and lost the case in fighting it because the mother bribed a doctor and got a note and also falsely lied in court. A PRO was issued for 6 months.
Now I'm trying my last option of an appeal because of things that went unfair in court, with the judge, and such. Things that would allow an appeal.
But I'm in the step with the Case Information Sheet and.... I don't get any what it's asking. I need help to explain what the questions are asking please.
I can't afford a lawyer to do it for me and I'm stuck doing all the legal forms and papers myself.

Are you referring to the form DV-120?

No. It was Civil Harassment RO. But I'm passed that already. I lost the hearing. Now I'm appealing it, and need help filling this page out http://www.courts.ca.gov/documents/app004.pdf
A lot of the wording and how they ask things I don't understand and need clarification.

To back that up I'd have to get into the whole story. And... I should warn you it is a very large wall of text. If it's needed I can. But can I get an answer on the paper before doing so? Mainly Section A, question 2. The way they worded that confuses me and I don't know what they're asking. This form is due the 18th though and I'll have to expedite it already. I need help asap.
