Birth Certificate Doesn't Match Social Security Card
My question involves name change laws in the State of: utah and ca
I was born in CA as Hadley, soon after my mother re-married as Schouten. She applied for a SS card for me as Schouten-she had proof with our medical card, BUT i was not legally adopted by my stepfather. I then got my Driver's license as Schouten (honestly without my Birth Cert saying Schouten I don't recall how). Needless to say I have lived my life since I was 2 yrs old, (now 40) as Schouten, I went to Jail at 18 as Schouten, served in the Military at 19 as Schouten, got a UT Driver's license and SS card as Schouten, worked always as Schouten. I lost my wallet 3 months ago and it has been a nightmare. The DMV wants a valid BCert and SS card with my proper name-I have Birth Cert-"Hadley" and no SSC, and all of my W2 and tax returns say Schouten. The SSA will accept my Birth Certificate, but require a legal name change to change my current records to Hadley (currently in their system I am Schouten). I can't get an ID or a replacement SS Card. What is the best way to handle-oh, I know about a name change but I would prefer not to be Hadley and it costs $600...Should I just go as Hadley now? and go through with an ID card as Hadley? If so SSA wants proof that I had a name change legally. What a mess! Help!

You would have to inquire with the SSA to see if they will issue you a Social Security Card under the name Hadley if you choose not to legally change your name.
