Changing Name Back to Birth Name
My question involves name change laws in the State of: Ohio
My birth name does not match my SS card. My mother in order to not get mine and my fathers cards mixed up just had my middle and last name put on my card. She later divorced my biological father and re-marrid and I was adopted and legally changed to his last name. The change was not completed with social security. She then divorced him after 9 years and re-married. We moved to another town and I was registered at school with her new last name and have been using that name since 1989 and filing taxes under my birth name. I now cannot renew my drivers license or obtain any state id. I have no valid id or legal documents with my birth name or adopted name and since the name on my social security card does not match my birth certificate 100% it get's rejected when I try to obtain any id. Without any valid id or documents how can I get my name and social security changed back to my original birth name? :confused:
