    Default Court Order to Amend Vital Records

    NY state

    I need a court order to get a certified copy of my great-grandparents marriage certificate for dual-citizenship. I also want to amend my grandfather's birth certificate (changing his mother's name from her nickname {not official name change} to her real name). The ONLY things I know are:
    1. Court order needs to be done by Supreme Court in NY.
    2. I need an index number and RJI in order to get the court order. Can I get orders for both of these things using the same index number or do I need one for each? ANY help is greatly appreciated as I have no clue what I am doing or even how to go about this.

    Follow the instructions here. If the respondent is the same you can probably get away with filing one case. But you're dealing with an esoteric request, and I expect a court to be very skeptical of changing a parent's name on a birth certificate, so I suggest working with a lawyer.
