My question involves a child custody case from the State of: north carolina
im 16 years old and i live with my mom through child custody and i want to live with my dad what kind of form do i need and can i still live with him if my mom does not agree

There isn't one. Your parents would need to go to court to modify the custody agreement.

If your mother does not agree your parents will need to go to court. Given your age (and level of maturity... whatever that is) the judge should take your wishes into consideration during the proceedings and will likely allow you to live w/ your father (unless there are some special circumstances)
