Adding Fathers Name to New York State Birth Certificate

My question involves paternity law for the State of: New York

My son is now 17months old and there was a mistake at the hospital where he was born and the father never signed the paper to be on the birth certificate (even though the nurses said he did). We now live in Alabama.

My question is how do i get his name added on and would it cost anything? And would i be able to do it from AL?
Having real hard time trying to do this. I dont know where to start....
