Father Did Not Sign Birth Certificate

My question is: I have had my daughter living with me since she was 9 months old. The mother (who has 3 other children) will not comply when I call and ask her for my daughter's birth certificate or social security card so I can get her enrolled in school for this fall (2009-2010). I have even told her that I will come to where she lives and meet her somewhere to get the information. She says she will call and let me know when and where to meet, but never a call. I have offered to come and do what I have to do to get my signature in the birth certificate. The reason the child is with me is because the mother cannot afford to take proper care of the child and has been investigated by Department of Social Services, at least twice; (for an incident with one of the other children that reside with her), since my daughter has been with me. What can I do?

You initiate paternity proceedings under the laws of your state. If that happens to be North Carolina, here's information.

I do have the Affiidavit of Parentage Form, so I can fill it out and take it to my Clerk of Court office here in Mecklenburg County or will I have to to take it to Guilford County where the child was born and the mother resides. I do have a copy of her current birth certificate (without my name on it). Please advise.

If both of you have signed the affidavit, and it's duly notarized, what information do you need?

The mother has to fill out her portion of the affidavit, once she has supplied that information, then we can have the paper filed?? Is there a fee for this process?
