My Ex-Wife Used My Social Security Number

My ex wife signed her name with my social security number and birthdate on a hospital document trying to tie me to a child who isnt mine.

what can i do about this?>

When did this occur? What was the result of her action?

The date of the documents were in July, a day before the birth of the baby. (she was induced)

I just recently found out about this due to a bill on my credit report. I dont know where to go from here. She actually had the baby in another city about an hour away.

Any ideas?
