Question Buying a House and 'Renting' a Room to a Roommate

i'm currently renting and will be buying a house soon. my roommate currently will be moving with me and we have agreed upon him paying me a set amount and also splitting utilities, etc. i've seen on this forum that this would likely be a ''lodger tenant''. i want to get this in writing so he can't just flake out on me and decide to move out without notice, etc.
My question is if what do i need to do to make any agreement i type up legally binding? should we go sign it and get it notarized at a bank or something? do we only need to type something up and agree to it? do i need to submit anything with the state, etc. if he'll be paying me to live there? thanks in advance for the help. this is in the state of wisconsin.

You can probably find form rental agreements at a local book store or office supply shop. There are a number of sites that sell them for download, although I haven't tried any of them.
