Default What Form Do I Need For a Workers Comp Claim
My question involves workers compensation law for the state of: Oregon

I have a shoulder injury and I have been off work for two weeks now since I cant move my right shoulder and I am right handed. And I might not be able to go back to work for a while. I asked my work for the workers comp form and they wont give it to me since they don't think it happen at work. Well it did happen at work and I have doctors notes and tons of paper works from them saying it is cause do to work and I need to get medical help and fine a new job. But at this time its really hard trying to fine a new job. I cant get what I need till I get the form and give it to my surgeon.
And my main doctors office doesnt do worker comp, so Im having a really hard time trying to get info and the form.
I even called the 1-800 number for my work so I could get the form and they wont give it to me. They said my work alone has to but my boss wont since she doesnt like me..
I need help!

It's your employer's job to complete and return the form. Try contacting the ombudsman.
