My child support order is in Arizona but I now live in Utah. Can I transfer the support order in Utah?

I'm also planning to modify the child support but my ex-husband is not so cooperative when it comes to giving his income information. How can I enforce him to provide all of his income information to me?

You are the support recipient? Has your ex-moved to Utah as well? If not, you may be able to move enforcement to Utah, but the former state's support formula would normally apply.

You can ask the court to order him to produce the missing documents, or try to get court permission to conduct discovery so you can compel its production by yourself (presumably with the assistance of your lawyer).
