Signing a birth certificate after three years

i was wanting to know how do i go about haveing my sons fathers name on the birth certifcate and my sons last named changed after my son is three years of age how do i going about doing this in the state of Indiana please get back to me i need to know thank you

Has the father acknowledged his paternity by virtue of an affidavit of parentage, or has he been adjudicated to be the child's father through a legal action?
