What is Required for Mother to Relinquish Guardianship of 16 Year Old to Another?

My question involves guardianship in the State of: Tennessee

Briefly, the situation is this: I need to know what forms and what the process is that my mom would have to fill out and go through for me to be able to legally live with my boyfriend's mother without fear of charges, etc. being brought up for me staying there. My father is currently in jail and will remain there for 2 years (according to his sentence). Will my mom relinquishing her rights as a parent suffice so that my boyfriend's mom can get guardianship of me? Thanks.

I'm 16 (see subject of post). And if my mom agrees to it, what difference would it make if I moved in with my boyfriend's family or not?
