Post Getting Paternity Established on Birth Certificate
My question involves paternity law for the State of: Maryland

My daughter is 8 months old, and when she was born the father was not involved so respecting his wishes I left the spot for the father's name blank on her birth certificate. He has resolved issues and now wants to be a part of my daughters life and wants to be on the birth certificate. I am just wondering what I have to do to get his name on the birth certificate and also what to do to get her last name changed to his.
