I doubt very seriously that the OP has standing to file a 397 with her ex's employer. She's made absolutely no statement regarding the fact that she has served upon him an I&E and he's refused to complete it (which is required in order to have the employer fill out the 397. She simply wants to subpeona the payroll records.
Now, if he's pursing support from her (which he should), then I&E's will have to be filled out anyway, so why the need to subpeona? The ball is all the way in his court at this point. He has 5 children in his custody, it is to his BENEFIT to fill it out, so I'm quite sure he will as soon as he serves her with the modification of support. If anything it would be her that I would worry wouldn't fill one out.
Will you have to pay support for the children that refuse to see you? Absolutely. They are still children, they still have expenses and they still have the right to be supported by both parents. You have alternatives that you can pursue in order to get your visitation, but I would highly recommend counseling instead to get down to whatever issues your kids have with you. To have 1 that doesn't want to visit maybe... but 2?
As far as a change in custody, you've said nothing that would pose a significant change in circumstance in order to warrant a change in custody.
If you wanted babies all to yourself, you should have created them by yourself. Until you do that, children have the right to BOTH parents, especially since you found them suitable to procreate with.
