Default Paternity forms
Is an affidavit of parentage and a decloration of paternity form the same the same thing? If not, can you explain the difference?

Also, if you sign one of those thinking someone is the father (assuming b/c we are not married we have to right?) but not knowing 100% is that illegal?

Default Affidavit of Parentage
Probably yes. States have different names for the affidavit by which a father confirms his paternity, outside of marriage. I believe the actual name for the form in Florida is "Consenting Affidavit Acknowledging Paternity".
